Social Services at Countryside Health Care of MilfordThe role of the Social Worker is to provide emotional support and counseling to residents and their families throughout their stay at the facility. Assisting residents and families to cope with the limitations an illness can place on one’s lifestyles is central to the social workers’ role. This is done through counseling and education.

The social worker provides information about community programs and services such as adult day health or assisted living. The social worker is also available to make referrals to appropriate home health care agencies for necessary services needed in order to return home.
