Nursing at Countryside Health Care of MilfordThe Director of Nursing, Nurse Managers, Registered Nurses, Licensed Practical Nurses, and specially trained Certified Nurse Aides form the nursing component of the Interdiscplinary Team.

Upon admission, each individual is fully assessed. Concerns and potential problems are identified by each discipline. Goals are established with the resident and his/her family toward optimizing each patient’s potential abilities. The Countryside Health Care Team consists of the Nurse Practitioner, Physician, Nursing Staff, Physical Therapy, Occupational Therapy, Speech Therapy, Activities, Food Services, and Social Services.

The Nurse Manager or designee functions as the coordinator and facilitator of the interdisciplinary team. The nursing staff provides daily care, medication administration, and reassessment of the effectiveness of care, medications, and treatments provided. They also play an integral role in working with the patient and their family regarding pain management, energy conservation, safety techniques, hydration, elimination and provide support and encouragement.
